Skuld thought of the Lenses... while she was programming reality.

Is that why she's so confident the Lenses can't be spoofed or copied? Because she integrated their workings at a level of reality that, if anyone else accesses it, Our Heroes have far greater problems than just fake Lenses? Why the Test Of Light is administered by the Lens itself, rather than being under Skuld's control, so that even if anyone does copy her access to that 'file' of Yggdrasil, it would still only produce 'real Lensmen'?

   This might also explain why her error of judgement only happened once. Hotfixes to reality and all that.

Well, no. She thought of them, and built the first one, when she first read the books that inspired them. She just dug them up again after the restart.

You're kind of both right, depending on your interpretation of "thought of". She'd made the first one, more or less on a whim, quite some time before the Ragnarök. She was definitely thinking about the Lens concept, and the impending need for it, while she was reprogramming the Engine, though; and so the OP's thesis is far from out of the question.
