"Default "View All"?"

Is this feature gone? I preferred this to table-of-contents mode, but it's turned off for me now and I can't seem to find a way to turn it back on. The FAQ doesn't have a relevant question for it, and there's no option in any of the user-configurable settings. And I can't remember how I first turned it on back in the day. Advice, clarification, please? Thanks. -- Bob
