The Terry Pratchett Discworld book "Feet of Clay" had two hardback first editions (UK and US) and then a paperback edition published by a third publisher (Gollancz, HarperPrism, Corgi respectively), but for whatever reason Corgi didn't have access to a manuscript while they were preparing their copy (possibly it was in the US with HarperPrism) and they used OCR. Clearly they had some problems with this because the first edition had a significant number of places where 'cl' replaced 'd' at the start of a word...
